Call Youth Hope at (909) 567-2808 to make an appointment date and time.
Call IEHP Member Services and state you’d like to make a self referral to behavioral health.
IEHP will ask for the following information: Doctor’s Name and Address and your appointment date and time.
IEHP will give you authorization within 5 minutes.
San Bernardino & Riverside County Social Worker Referred. Ask your social worker for authorization.
Health Net Plan Members: Call to make an appointment. Bring your member id card to first visit.
